OPERATION ZERO

Policies and Procedures

LEGAL

Prior to initial participation in Operation Zero’s 8-week injury risk reduction programs (the “Program”), all athletes or parent/guardians (if athlete is under 18) must complete and sign required intake and liability forms provided by Operation Zero (the “Organization”).

To ensure adequate screening time, all forms must be completed one week prior to the Initial Data Collection Day (start day).  Participation cannot be guaranteed for those who do not meet this deadline.

Athletes under 18 years old are required to have a parent/guardian signature on all forms.

The Organization and its staff, officers, agents, and employees (together, its “Representatives”) do not provide injury diagnosis, physical therapy evaluations, treatment, and discharge planning.  If you have a condition that is beyond the scope of the Program, we may suggest you consult a physician 
or obtain a referral for a physical therapy evaluation.


FINANCIAL


The Organization charges a fee (the “Fee”) for athlete participation in the Program.  The Fee is $169 per youth athlete (age 11 to 18) and $199 per adult athlete (age 18+) per 8-week Program.

For those athletes who plan to attend the Initial Data Collection Day (start day), the Fee is due, in its entirety, one week prior to that day.  

For those athletes who are unable to attend the Initial Data Collection Day (start day), but still wish to participate in the Program, the Fee is due, in its entirety, prior to Program participation.

If Fee is not paid, Organization reserves the right to suspend Program.

Family/Athlete is responsible for the Fee and any additional elective fees (see Elective Services and Fees for details).

PARTICIPATION


A minimum of 13 athletes must complete registration and payment one week prior to the start day to secure team participation.

To ensure program quality and safety, a maximum of 18 athletes per team may register.  Exceptions may be considered at the discretion of the Organization.

Due to limited time and resources, only one Initial Data Collection Day (start day) and one Final Collection Day (end day) can be offered to each team.

An athlete who is not present on the Initial Data Collection Day can still participate in the program if all forms are completed and signed and the Fee is paid, in its entirety, prior to Program participation.

An athlete who participates in the Program, but is not present on either (i) the Initial Data Collection Day or (ii) the Final Collection Day, will have an incomplete analysis at the end of the Program.

Family/Athlete will not receive a refund if athlete does not participate in complete Program for personal reasons including, but not limited to illness, injury, holidays, vacation, transportation, etc.  

The Organization reserves the right to accommodate a team if team does not participate in complete Program for non-personal reasons including, but not limited to local, state, or national emergency, local weather alert, unsafe field conditions, etc.

The Organization reserves the right to refuse services to any athlete, family member, or coach who is past due on Fee payment, exhibits unsafe or disruptive behavior, and/or is the subject of disciplinary action by the team, club, league, or any organization that the Organization is affiliated with.

ELECTIVE SERVICES AND FEES


Due to the potential complexity of data analysis and high athlete participation volume, any summary analysis and/or educational materials provided to coaches, athletes, and/or parent/guardians will be shared in written form (ie. email).

Individual phone consultations are not included in the Fee, but may be requested by email.  Phone consultations are not guaranteed, but may be arranged for an additional fee at the discretion of the Organization. The Organization reserves the right to waive or alter the fee for reasons including, but not limited to, complexity of information, consultation time, availability, etc.
